Vivid Care Service is recruiting an Adult Social Worker to work on their Palliative Care Team. Personalised Long-Term Support Pathway delivers a range of services for adults and older people, generally over the age of 65 years old. This service includes the Palliative Care team. For Older people, the service is delivered by a team of social workers and service Coordinators providing assessment and support planning. The team includes a Reviewing function and by way of a Reviewing officer that focuses on timely and purposeful reviews. The team enables staff to develop knowledge and skills in working with a mixed caseload and the opportunity to contribute and projects as their career progresses. The Team management will develop the Adult Social Care vision and embed the operating model based on strength and asset-based approaches.
