Outreach support is a flexible, community-based service where a dedicated Vivid Care support worker accompanies an individual into the community — or visits them at home — to help them live the life they choose. It is not about fitting people into a programme; it is about building a life around what they love.
Every session is built entirely around what the individual wants to do, where they want to go, and what matters most to them.
